Membership Agreement

If my membership dues are paid through Credit Card draft or Electronic Funds Transfer, I understand this is a continuous membership plan. This membership will remain in effect for as long as I maintain current payment. Membership cards are the property of the YMCA and must be surrendered upon request. It is my complete understanding that if I wish to terminate or change my continuous membership plan in any way, I must give the YMCA written notice by the 28th of the month.
 

All membership rates are subject to change with 30 days written notice. I understand it is my responsibility to notify the YMCA of any change in address, bank account information (if utilizing bank draft for payment of dues) or credit card information /expiration date (if utilizing credit card for payment of dues).
 

The Joiner Fee is a one-time fee as long as the membership is current and continuous; you will never pay this fee again. If you choose to cancel or discontinue your membership for more than 45 days, a Joiner Fee will be charged when you reapply for membership. Joiner Fees are non-refundable.
 

The Ed & Ruth Lehman YMCA will not assume responsibility for any injury incurred during participation in any YMCA programs or events including childcare, sports, aquatics, or fitness programs. Certain risks are inherent during participation in these events and membership related activities.
 

Nor will the YMCA be liable for lost or stolen items while members and/or program participants are using YMCA facility or are on the YMCA premises. I, the undersigned for myself my heirs and assigns, do hereby release the YMCA and its employees, volunteers and agents from any and all claims for injury death, loss or damage I may suffer as a result of my participation.
 

I give my permission to the Ed & Ruth Lehman YMCA to use, without obligation, photographs, film footage or tape recordings which may include my image or voice for purposes of promoting or interpreting YMCA programs.
 

I have read and agree to comply with the above agreement. I further agree that I (and those on my membership) will uphold the mission of the Ed & Ruth Lehman YMCA. I/we agree to demonstrate the YMCA core values of Caring, Honesty, Respect and Responsibility.
